Client Intake Form

When you book an appointment, a client intake form will be sent to you via email or WhatsApp. This includes:

Your general and contact information, medical information and a statement of release.

Cancellations
We understand that things may happen. If you need to cancel or reschedule your appointment, please let us know as soon as possible.

Late Arrivals

If you are running late, call and let us know. We will try our best to perform the service in the remaining allotted time. This is not guaranteed.
